Freeze Policy: Member may place their membership on hold for 1 to 3 months each year by providing written notice to the club 10 days prior to the hold. Member has an option of two types of freeze requests. Time Freeze in which the monthly dues would continue to draft during the freeze and the months added as credit at end of term. This way allows member to choose the specific date range in months for the hold. Payment Freeze in which the freeze must line up with members invoice cycle. Keycard would be deactivated in line with the invoice cycle.
Thank you for being a valued member. We are sorry you wish to stop your membership. Please note, all monthly dues will continue for 30 days, after which, your Membership will be Cancelled provided all requirements to cancel have been met. Members are required to make all payments that are due within these 30 days. All accounts must have a zero dollar past due balance for a cancellation to be processed. Members must fulfill all monthly commitments established at the time of joining before membership cancellation will be processed.
Cancellation Policy: All cancellations require a 30-day notice. Member may only cancel within the initial term for two reasons: Medical (Dr.’s note required) or moving purposes. Member may cancel membership if residence is permanently moved more than 35 miles from the nearest facility location. Member must provide official written documentation of change of address. Important Notice: A $40 cancellation fee will apply to memberships cancelled within the initial term. The Member may cancel the Agreement and receive a refund if Club goes out of business, moves its facilities more than 10 driving miles from 2221 East Ridge Center, Eau Claire, WI 54701 or 2405 Folsom Street, Eau Claire, WI 54701 or fails to provide within 30 days a facility of equal quality at no additional cost to the Member. The Club shall not be deemed out of business when temporarily closed for repair and renovation of the premises, for not more than 14 consecutive days, and not more than 2 periods of seven consecutive days in any calendar year.
Submission of this form is a request to cancel which you will receive an email copy of this submission. Once confirmation of terms and conditions are met we will process the cancellation and sent a confirmation email of completion.
